DATA SECURITY: 

The Life Bridge Ministries processes information on our servers located inside and outside the United States; therefore, your Personal Information may be processed on a server located outside of the country where you live. We have implemented physical, administrative, and technical safeguards and security measures we consider reasonable and appropriate to protect against the loss, misuse, and alteration of the Personal Information under our control. However, no data transmission over the Internet or any wireless network can be guaranteed to be 100% secure; therefore, we cannot guarantee or warrant the security of any information you disclose or transmit to us online and you do so at your own risk. We are not responsible for the theft, destruction, or inadvertent disclosure of your Personal Information. In the unfortunate event that your information is compromised, we will notify you in the most expedient time reasonable under the circumstances. We may delay notification as necessary while we take measures to determine the scope of the breach and restore reasonable integrity to our systems as well as for the legitimate needs of law enforcement. From time to time we evaluate new technology for protecting information, and when appropriate, we upgrade our information security systems.

 

Our company is hosted on the Wix.com platform. Wix.com provides us with the online platform that allows us to sell our products and services to you. Your data may be stored through Wix.com’s data storage, databases and the general Wix.com applications. They store your data on secure servers behind a firewall.  

 

All direct payment gateways offered by Wix.com and used by our company adhere to the standards set by PCI-DSS as managed by the PCI Security Standards Council, which is a joint effort of brands like Visa, MasterCard, American Express and Discover. PCI-DSS requirements help ensure the secure handling of credit card information by our store and its service providers.

Our Site may contain links to other websites operated by third parties. This Policy does not apply to your use of third-party websites. The Life Bridge Ministries is not responsible for the information practices or the content on such linked sites, and your use of those third-party sites is not governed by this Policy. We have no liability or responsibility for those sites or their policies/practices. We collect such Non-personal and Personal Information for the following purposes:

To provide and operate the Services;

To provide our Users with ongoing customer assistance and technical support;

To be able to contact our Visitors and Users with general or personalized service-related notices and promotional messages;

To create aggregated statistical data and other aggregated and/or inferred Non-personal Information, which we or our business partners may use to provide and improve our respective services; 

To comply with any applicable laws and regulations.

Data collected and stored by vendors, suppliers, third parties, etc. are outside the responsibility of the company in terms of access, storage and control.

Access to data is granted to owners, and specific personnel as appropriate, based on the business needs, while taking into consideration the legal requirements that also apply to data access.

The company should keep all original and corrected information for the required retention periods, based on the data type collected, taking into consideration any legal requirements that may extend retention times beyond the standard retention period, currently 4 years.

We may automatically collect and temporarily store information related to your visit to our website, mobile apps, or other media platforms that is not personally identifiable information.

We do not collect personally identifiable information about you unless you choose to provide that information to us, or it is collected as part of conducting normal business-related activities.

Any personally identifiable information you choose to provide is protected by privacy and security practices.

The Life Bridge Ministries does not disclose, give, sell, or transfer any personally identifiable information about our visitors unless required for law enforcement or by federal law, with the following exceptions:

For Counselling and Intensives related visitors, nothing is shared or disclosed.

For Workshops, Conferences, Speaking Events and Retreat visitors:

Information required as part of registration and attendance is shared with the third-party apps used in the registration process, and limited information is shared with personnel responsible for the event coordination and event related activities.

Our ability to edit/remove posted material and comments on our social media platforms and other promotional platforms may be limited.

If you do not want your images, videos or audio recordings shared on our social media platforms or other promotional materials, you must OPT OUT in writing before the event begins and notify the event coordinator so items can be verified prior to posting for compliance purposes.

Images, audio or video recordings of the event at large may be taken at the event and shared for promotional purposes.

Images, Audio or Video recordings of individual testimonials may be taken, with permission from the individual, and shared for promotional purposes.

For Newsletter subscribers, information required to send the newsletter is shared with the third-party app, as well as personnel responsible for the newsletter.

You may unsubscribe by clicking on the unsubscribe link at the bottom of the newsletter.

For Social Media Platform subscribers, followers, and viewers:

Information is shared with the various social media platform apps, as determined by the platform itself, based on viewer type. This sharing is outside of our control.

The Privacy of the page and contents posted to the page is based on current app platform settings and is subject to change at the discretion of the app platform.

Pictures, videos, comments and tags by non-staff members or volunteers on our social media platforms are outside of our control and responsibility.

You may unsubscribe or unfollow the social media platform at any time. However, all posts and comments remain on the platforms.

For payment processing purposes:

Cash and checks are handled by appropriate personnel.

No banking or credit card information is stored by us or the third-party financial institution (i.e. bank or credit card processor) except for what is required for immediate use only for purposes of conducting the transaction required. Banking deposits are done with a deposit banking app. Checks are filed after deposit for at least 30 days. Credit card processing transactions are done via secure networks, controlled by the third party.

The Life Bridge Ministries does not disclose, give, sell, or transfer any personally identifiable information about our applicants, volunteers, staff and board members, unless required for law enforcement or by federal law, with the following exceptions:

For access to required business related software or information related to the specific activities the individual is responsible for, limited information may be shared with appropriate personnel for access, setup, security and conducting business related activities.

Limited information may be shared, as appropriate, with appropriate volunteers, staff, board members, subcontractors or third parties, as required, as part of conducting business-related activities.

We may contact you to notify you regarding your account, to troubleshoot problems with your account, to resolve a dispute, to collect fees or monies owed, to poll your opinions through surveys or questionnaires, to send updates about our company, or as otherwise necessary to contact you to enforce our User Agreement, applicable national laws, and any agreement we may have with you. For these purposes we may contact you via email, telephone, text messages, and postal mail.

 

THESE ARE YOUR RIGHTS:

You have the right to limit use, request deletion or disclosure (partial or full) of sensitive PII by the business of “publicly available” information. For non-publicly available information, you can request we limit use of information for as needed to provide specific business purposes only.

You have the right to access/view your information we have on file, with management present, based on current privacy and legal rules regarding access.

You have the right to request corrections/updates to the information as appropriate. Requests should be made as soon as you become aware of the need for updates/correction.

If you desire physical copies of the information in question, you must pay for applicable expenses (copies, the time involved in making copies, shipping).

You may initiate a request by phone. However, you must also submit a formal request in writing, submitted to the appropriate department. Texts do not qualify as a suitable form of writing regarding requests.

Requests for payroll related information must be made separately from all other request types.

Requests for employee records must be made separately from all other request types.

Requests for HIPPA/ restricted records must be made separately from all other request types.

Management will respond to your request within applicable timelines, whether within the original applicable timelines for the request type, or within approved extensions as appropriate.

Regarding any requests for deletion of data, if there are any exemptions applicable to delete requests, management should indicate when the basis to delete is partially applied/partially exempted.

We may decline to process requests that are frivolous or unreasonably repetitive, risk the privacy of others, or are extremely impractical. If you need help in preparing your request, please contact our office.

Volunteers, Employees, Management and Board Members will maintain confidentiality to the extent that is feasible, for all information seen/used while doing business as well as any requested information.

The Life Bridge Ministries may from time to time make changes to this Policy to reflect changes in legal or regulatory obligations or changes in the manner in which we collect and use Personal Information. Legal and Government entities should submit written requests per their procedures to the appropriate executive office/officer. Any Legal forms submitted with the request must be completed and submitted per timelines and instructions indicated. The Life Bridge Ministries Personnel will contact the requesting entity with any questions regarding the form and any additional information requested.

Management may ask for additional information in order to respond to the request. This may include verification of identity, verification of authenticity of request, as well as information regarding the request itself.

 

How The Life Bridge Ministries Management will Respond to Requests For Access, Updates, or Corrections: (This part can be an internal policy document only)

The company must make reasonable efforts to determine the validity of the requestor and the written request, and potential legalities associated with the request prior to responding to request.

Must use reliable data points to verify ID of person. Such items include information the company may have about them on-file.

CANNOT USE NAME, CONTACT INFORMATION, and HOME ADDRESS as sole

information for verification as this may be publicly available and is subject to breach.

If have a password protected account, can use the existing account as verification.

Cannot create a new account for CCPA purposes only.

DO NOT RESPOND TO REQUEST UNTIL YOU ARE ABLE TO VERIFY ID AND AUTHORIZATION FOR REQUEST.

REQUEST AND RESPONSES MUST BE IN WRITING.

Requests cannot be made to access another individual’s information without the express legal consent appropriately given by the individual for the specific information being requested, except as permitted by law.

HIPPA and CCPA privacy rules apply and may result in limiting the information that is shared or the manner in which it is shared. Any PII information related to HIPPA, an incident, or investigation must include legal for determining what can be shared, changed and/or discarded.

Specific forms may be provided by legal and government agencies. These forms must be completed and submitted according to the timelines and instructions given with the request.

Contact the entity requesting the information with any questions or concerns.

The company must make reasonable efforts to make available, update or correct PII information within the appropriate timelines, based on the information being requested, provided the request is legal and valid. Unless specific timelines apply for the request type, do the following:

Respond within 10 days of written request your acknowledgement of request in writing.

Provide requested information within 45 days. If you require an extension of an additional 45 days, you must alert in writing the need for extension and get approval.

Management may initiate required forms on behalf of the employee per employee’s request, but the employee must review, sign and date the forms for the forms to be considered “qualifying”.

All original and signed documentation is filed by HR.

Volunteers, Employees, Management and Board Members will maintain confidentiality to the extent that is feasible, for all information seen/used while doing business as well as any requested information.
